Jimmy Smith Academy :: Registration Form / Emergency Consent
At the Jimmy Smith Academy safety is our first priority. Accidents may occur from children's participation in youth sports activities. We ask that for any parent wishing to have their child participate in the Jimmy Smith Academy download and complete the registration and emergency consent form as provided. This emergency consent, for the treatment of minors, needs to be completed by the parent/guardian so that your child can be treated even if you are present or out of contact at the time the emergency occurs.
