Team 82 Football League : Registration Form / Emergency Consent
At Team 82, safety is our first priority. Accidents may occur from children's participation in youth sports activities. We ask that for any parent wishing to have their child participate in the Team 82 programs, download and complete the registration and emergency consent form as provided. This emergency consent for the treatment of minors needs to be completed by the parent/guardian so that your child can be treated even if you are present or out of contact at the time the emergency occurs.
